communityOur experienced health educators provide educational presentations on a variety of topics, including family planning, HIV/AIDS and STDs, diabetes, nutrition, hypertension and dental health.

 

Santa Cruz County: Salud’s Community Outreach Department is active in the community providing education, program enrollment assistance, point-of-care service, as well as providing services under the terms of grants which the organization has received, to better serve Salud’s patient population. Outreach personnel are located at 204 East Beach Street, Building B, Watsonville, CA 95076, adjacent to Salud’s Main Clinic, and at Salud’s Clínica del Valle del Pájaro, at 45 Nielson Street, Watsonville, adjacent to the Watsonville Community Hospital.

 

Education: Salud’s experienced health educators and outreach workers give educational presentations to community organizations, students, school staff, and parent groups, on a variety of topics, including diabetes, nutrition, obesity prevention, healthy living tips, dental health, hypertension, family planning, HIV/AIDS and STDs. Presentations have also been given to migrant and seasonal farm workers. In the peak of the harvest, at-work presentations are given to seasonal farm workers about the community’s health care resources and health topics which are relevant to farmworkers’ well-being.

 

Program Enrollment Assistance:

Medi-Cal: Staff assists individuals in applying for California Medi-Cal assistance.

Healthy Families/Healthy Kids: Staff assists in finding health insurance assistance for children of low-income parents who may not qualify for no-cost Medi-Cal. The Healthy Kids program has been available for residents of Santa Cruz County since the inception of the program.

Medi-Cruz: Staff assists individuals in enrolling in Medi-Cruz Advantage, a Santa Cruz County operated health care program that helps low-income residents of Santa Cruz County without health insurance get the health care services they need.

Patient Assistance Program (PAP):  Outreach assists qualified individuals in obtaining affordable prescription medications, by working with the pharmaceutical companies who sponsor these programs.

Vision Prescription Assistance for Children & Seniors (VSP):   Staff helps children and seniors qualify for eye prescription assistance which will pay for eyeglass frames and lenses.

Family PACT: Staff enrolls individuals in Family PACT, a program which provides no-cost family planning services to men and women, including teens.

Food Stamps/ CalFresh: Outreach staffers assist qualified individuals in enrolling for supplemental food stamps, now known in California as the CalFresh Program.


Point-of-Care Service:

  • HIV Testing
    Community outreach staff has been certified by the State of California to conduct free HIV anonymous testing and counseling.  Tests are available on a walk-in basis at the Beach Street Outreach Office in Watsonville, and at Salud’s Beach Flats clinic in the City of Santa Cruz; results are available within 20 minutes.   • Food Distribution
    Held every Wednesday at 1:00 pm at Salud’s Main clinic, 204 East Beach St., Watsonville, this free
    food distribution has been a tremendous benefit to our community. This is a collaborative effort between the clinic and Second Harvest Food Bank to ensure that Salud's patients and the low-income population receive the proper nutrition. Distribution is on Wednesdays at 1:00 pm, rain or shine. No appointment is necessary.

  • Champions for Change
    At Salud we place a high value in helping our patients access information that leads to healthier lifestyles.  We provide on a regular basis nutritious recipes and newsletters with nutrition information.  Don’t forget to pick them up when you visit us!

  • Examples of Grants under which Outreach is working:

     

    CVHN: With grant support from the Central Valley Health Network, Salud is working to address obesity and diabetes rates in the Watsonville area. Salud offers group adult/youth/child educational nutrition talks that discuss basic nutrition, portion distortion, understanding the basics of nutrition labels, and becoming a conscious shopper/eater. Furthermore, as the majority of staff live within the community Salud has implemented an Employee Wellness Program. This program looks to improve staff health by addressing nutrition, physical activity and stress reduction.

     

    La Vida Sana: Grant-supported Spanish language program offering group classes and gatherings focusing on health education, healthy eating and healthy cooking.

     

    Monterey County: Community Oral Health Services, a program of Salud Para La Gente (COHS) has outreach staff headquartered in Salinas and Greenfield that reaches out to and has established collaborative relationships with many local partner organizations in Monterey County.  COHS outreach also conducts dental screenings, and provides dental education to children, parents and teachers.

     

    San Benito County: As Salud assesses unmet health needs in San Benito County, and opportunities to serve, its outreach staff is exploring participation and potential collaborations with partner and host organizations, in a variety of contexts, including health fairs, screenings, school health facilities and health education presentations.
