Do I need an appointment?

If it’s an emergency, call 911 right away. But if you have other health concerns, it’s always best to get an appointment. Just call (831) 728-0222. You may be able to come in the same-day.

How do I make an appointment?

It’s easy. Just call (831) 728-0222 to make an appointment. You can get care at any of our five clinics or six School-Based Health Centers.

What time do I need to arrive for my appointment?

Try to get here 15 minutes early. That will give you time to fill out any needed forms.

What if I am not able to fill out the forms?

At Salud, we can help you fill out any needed forms. Just ask us. We’re glad to help.

What do I need to bring to my appointment?

It’s a good idea to bring:

  • Your insurance card or proof of insurance, if you have insurance.
  • Any meds you take.

What if I don’t have insurance or any money to pay?

  • If you do not have health insurance, be sure to tell us. We can help you apply.
  • Even if you are unable to pay, you will not be turned away.

Do I need to live near a Salud clinic to receive healthcare services?

No. At Salud, we serve everyone, regardless of where you are from or where you currently live.

Do I need a doctor's referral to receive healthcare at Salud?

No. You do not need a doctor’s referral to be seen in our clinics.

What is specialty care?

Once you are a Salud patient, you can get specialty care like help with back pain and skin problems.

How does Salud take care of medical records?

At Salud, we know how important your privacy is. We are committed to keeping your records private. We keep these records so we can:

  • Give you quality healthcare.
  • Get paid for the services we give you through your healthcare plan.
  • Make sure that we meet the standards of care called for by law.

By law, we must:

  • Make sure to protect your medical records.
  • Tell you how we plan to keep your records safe.
  • Tell you right away if your protected information has been shared by mistake.

When you call (831) 728-0222, our patient access representative will talk with you about how to choose your family’s doctor. You have many to choose from! Here’s a list of those dedicated people who work here at Salud.

Payment Questions

What co-pays can I expect?

It depends on your insurance.

  • If you have a co-pay, you can pay with cash, check, debit card, or credit card.

If you don’t have health insurance:

  • We can help you apply for insurance if you qualify.
  • Or you can pay on a sliding fee scale.

If you pay on a sliding fee scale:

  • Your medical visit can cost $20 to $80.
  • Your routine dental visit can cost $40 to $60.

Can Salud help me pay for the care I need?

  • We will give you our services whether or not you can pay.
  • Salud will help you apply for health insurance.
  • We can link you with the help you need.
  • If we cannot find a program for you, we have a sliding fee scale.

What if I have an HMO Health Insurance?

Salud only accepts PPO plans.

What if I don’t have insurance?

You do not need to have insurance to come to Salud. If you want to get health insurance, we can help you apply.

I want to apply for insurance.  What do I do?

Call us at (831) 728-0222. Tell our patient access representative you would like to apply for health insurance. You will get an appointment to come to apply.

What forms or information do I need to apply for insurance?

When you come to apply, please bring:

  • The check stubs for one full month for anyone in the family over 19 years of age. Or a copy of your latest tax forms.
  • The social security cards for anyone in your household who wants to apply.
  • Birth certificates or passports for anyone in your family born in the United States. Or the Permanent Residency Cards for those born in Mexico or in another country.
  • Proof that you live in Santa Cruz County or Monterey County. This could be a phone bill or a gas and electricity bill. You could also bring a letter with a street address that is less than 45 days old.

Financial Support Questions

What other kinds of support can Salud help give?

  • We can give you information about transportation resources, so you can get to your appointments.
  • We can give you referrals to other Community Agencies, to help your family stay healthy.
  • We can help you apply for CalFresh so you can get extra money for food.
